Employee Assistance Program contract disclosure

Like many professional and caring organisations, DJJ has an employee assistance provider. ITIM Australia (now Converge International) came on board on 1 January 2006 and under the Premier's direction (Memorandum 2000-11), DJJ has provided particulars of the contract as below.

PART 1 – CONTRACT DETAILS


Contract Name/Description:	Provision of Employee Assistance Program (EAP)
Basis of Payment:	Firm for initial twelve (12) months then subject to annual adjustment of 3%.
Contract Manager:	Stephen Barton
Contract Duration:	Three (3) years with two (2) twelve (12) month optional extension periods.
Contract Conditions:	Standing Offer Agreement.
